Horizon Rehabilitation and Sports Medicine is a provider of outpatient, orthopedic rehabilitation. Utilizing the skills of physical, occupational, and massage therapists, as well as athletic trainers and personal trainers, Horizon promotes the prevention, care, and rehabilitation of orthopedic, sports, work, and occupational injuries. Horizon has expanded its sercvices to included over 15 different programs.

Horizon's business staff will assist you on an individual basis filing all insurance claims. In addition to accepting numerous insurance plans, Horizon is also Medicare certified.
